The "default gateway" is your router.
You access your router's configuration page. Then, you go to the page that has to do with the forwarding (Gaming,Applications,Security,etc). In there you type the port you will forward, TCP or UDP, and for which IP address.
The subnet mask within the TCP/IP configuration is used to distinguish the ? from the host address.

the tcp/ip frame uses the ip address to send messages between devices (computers, printers, etc) on your network. This frame is inside an ethernet frame on the network layer.
