I would suspect a weak EGR valve. It should be toward the rear of the motor. Take a non or low flammable spray (WD40) and spray on or around the valve and on and around the seals and if the engine smoothes out that is where the problem is. A severe vaccum leak causes a lean condition which in turn causes the problem you have. Also spray around the hoses, gaskets, etc.

Your piston rings are probably bad. Which will cause compression loss, in effect you will loss power, and it will let oil trough and saturate your plugs.
