water pumps in malibus can fail because the plastic impeller can spin on the steel shaft. If the water pump is ok then it is probably a head gasket.
Lots of reasons, but mine was the fuel pump was bad. It would overheat after driving maybe 15 minutes, then I would let the car stop for an hour or two and it would start back up.
Check the hose going to your pcv valve. If it is split or not connected good, this will cause the high rev. Did it on my 1998.
