Most likely, when they were installed last, they were over-torqued. Another possibility could be excessive rust on the wheel studs.
Most likely they are cross threaded because of the wrong thread pitch on the lug nuts. But could just be over torqued.
There are a couple of different tools that are good for tightening lug nuts. A torque wrench and a lug wrench are good for tightening lug nuts. Typically lug nuts would be tightened with a torque wrench, but a lug wrench is much cheaper.
Loose or broken lug nuts. If the lug nut were installed in reverse they will also come off.
Locking lug nuts are just as strong as standard lug nuts, just as long as they are correctly installed with the proper locking lug nut key wrench.

Remove old lug nuts replace with new one. I recommend buying lug nuts from toyota dealers. OEM lug nuts last longer than aftermarket.
That car may have locking lug nuts if they are the factory originals. You would need the "key" for the lug nuts in order to remove them.
