If its slipping only in 4th gear, it could be that your releaser bearing is gone with the p/plate. Or your gearbox has had its days and would need to be replaced/ repaired.

A large vacuum leak could effect throttle position, but it would cause other problems more noticable than the effect on transmission shifting. a vacuum leak mpuld not cause trasmission to slip but a band inside transmission would cause the problem
Yes, if the fluid level is high enough it can aerate and that can cause it tp slip.
