Damaged catalytic converter is one possibility.
The purpose of an accelerator is the car is to give the engine gas so the car will move.
Push on the accelerator. Turbo charge the engine for more power.
For a gas engine, the accelerator opens a valve in the carburetor/intake manifold, which allows more air into the engine. With more air, the carb/injector will deliver more fuel. WIth more air and fuel going into the engine, the engine will make more power.
I think that happens when you are changing gears. If you make the engine goes to 3000 rpm and suddenly release the gas pedal and rapidly press down the clutch pedal. The rev would up. With very noisy sound. Isn't it?
Flat spots occur due to several reasons. It could be leaks in the accelerator pump piston or the accelerator pump valves. It can occur when you are accelerating because there is a momentary hesitation from the engine after the pedal is pressed down.
There are so many things I would need to know to answer this question. Make model year engine, other symptoms, etc. It needs to be checked by a knowledgeable person.

If the engine runs well after it's warmed up, you just need to adjust the choke to make the engine start and run better when it's cold.
Your engine needs more gas to accelerate. Either your accelerator pump is worn out or it needs to be adjusted. Could also be timing. But more likely the accelerator pump. Look down inside the carburetor to make sure you're getting a good healthy squirt of fuel when you pump the gas. Make sure the car isn't running when you do this.
I would first look at the accelerator pump in the carburettor. The accelerator pump puts a short burst of raw gas into the air stream. This gives an enriched air flow into the engine causing acceleration. To check the accelerator pump: With the engine off remove the air filter and hold the choke valve open (if it is closed). Then move the accelerator linkage to open the throttle valve and you should be able to see a spray of raw gas into the venturi. If there is no spray then you need to make sure the linkage is working correctly. If the linkage is pushing the accelerator pump down and there is no spray you may consider replacing the accelerator pump.
You would apply force to the accelerator to make the car go faster.
Does the engine rev when you press on the accelerator? If it does check to make sure the emergency brake is not engaged. Is the car an automatic or a standard transmission? You have to make sure it is in gear.If it is a standard the problem is the cluch apparatus, pressure plate etc. If automatic you have to check the level of fluid in the automatic transmission.
