Most common causes include the plug wires, distributor cap and rotor, fuel pump and fuel filter. Fuel delivery being the most likely suspect since it is not happening across the rpm band.
If pieces of the broken cat are rattling around in there then yes. If this vehicle is a 1996 or later and the cat is broken the check engine light should be illuminated and the codes should point to a failed cat converter.
It can be something that has changed around him so he gets the stress out this way. Make sure you have him checked physically first.
Maybe but more likely its the other way around. Oil blowby will ruin the converter quickly. Perhaps if the check engine light was on & the engine quickly replaced then the converter would still be good. A clogged cat will just rob performance & the engine might not start. The oxygen sensors abre probably ruined from the oil blowby too.
It sounds like your "Timing Belt" broke. Have it checked, if it's broken it should cost around $200 to have it replaced on your six cylinder L300.
The 2002 Mazda MPV has two flex pipes that comes from manifold. One of these flex pipes comes with the catalytic converter. I had it changed at Speedy and it cost around $800.00 cost, labour,taxes incl.
If there is nothing loose rattling around inside and the transmission fluid is clean and the surface where the seal rides hasn't worn down, the torque converter MIGHT be ok to use. You're taking a gamble and you'll have to decide if it's worth the risk. I'd rather spend a couple hundred on torque converter now, then grand on the transmission when the converter fails.
My 2002 had the same issue. After trying the obvious, replacing air filter, plugs, fuel filter, etc. I had it checked and the catalytic converter was going bad. I found a bolt-on converter on usautoparts.com for about $120. Most parts stores I looked at around here were $250 and up.

the engine is a 5. liter (302). when i say lag i mean it only reve to around 2200 rpm and then powers out. when I push in the clutch the rpm's stay around 1600 rpm. I checked the bolts on the plemum (intake) and they are tight. the throttle linkage and cable doesnt stick . I replaced the fuel pump and filter. The check engine light doesn't come on . I checked all the vacuum lines and all appear to be fine. I replaced the cap and rotor, wires and plugs. Air filter is ok
