Yes, it is terribly bad and wrong to melt the butter first. Creaming butter and sugar together is just smashing them together until a butter and sugar paste forms, but the butter should remain cold.

I would say it depends on how far ahead of time and what kind of butter / margarine you are using. If you are using regular butter, you would probably be fine, just put the mixture in the fridge until 1/2 hour before you will be using it (be sure to cover it tightly). If you are using something like margarine or spread that tends to separate, the results might not be quite as good. However, you will be beating it up again when you add the rest of the ingredients, so I suppose that might take care of any separation. In general, I guess you could, although I don't see the advantage. You will want a mixer when you add the egg, etc. also, so it doesn't save you having access to a mixer at a later time.
