They don't "cut it off" - an outtie becomes an innie: because the penis is a tube - it can be inverted and this is what is generally done. The penis is adjusted and the muscles are cut to allow the inversion. The patient retains the original penis (sans a few small parts) - it now is used as the basis for a vagina and sewn into the abdomen at the correct location.
