There have been numerous oil spills, both offshore and inland. The worse oil spill occurred as a result of a broken pipeline, in January 2000 resulting in contaminated beaches in Rio de Janeiro, Guanabara Bay. More oil spills on Guanabara Bay occurred on July 25, 2000.
Also, in November 21, 2001, the beaches in Guanabara Bay were again polluted by the oil spill from the Manguinhoa refinery.

Inland locations have had spills, including the sinking of the barge in the Amazon river in Feb 7, 2000. Fortunately, the impact was reduced by quick action by authorities (see first link).
On July 16, a refinery oil spill resulted in pollution in the Barigui river, Curtiba, in the southern area of Brazil. This river flows into the Iguacu river.
Oil spills in Brazil have numerous sources (barges, pipelines, tankers, refineries and offshore platforms), and many locations ranging from Curitiba to the Amazon. Certainly, the most environmentally damaging (so far) would be in Guanabara bay.
