Step one is to go to the police. If you don't know how to contact them, pick up the phone and dial 911. Say you would like to report a domestic dispute and that you would like to remain secret as far as your identity is concerned. This way you don't need to worry about anyone coming after you.

Does a child have to go with non custodial parent when they are being physically abused by that parent?

Children have the right to live where they are safe. If a child, or you, are abused you call the police or the Child Protection Agency. The court will give custody to a relative or if one can not be found, a foster home.Answer and ClarificationNo. But as the custodial parent you are responsible for protecting your child. You need to return to the court that has jurisdiction immediately and request temporary orders to modify visitation until the court can investigate the situation and terminate the visitations.

Yes and no. Without addressing the allegation that she is being "abused" - she can travel out-of-state with her child. HOWEVER - she can not do so with the intent of deserting the father and depriving him of any contact with their child. He, after all, is a parent and has rights as well.

Yes, if conditions exist that show the mother is an unfit parent, the child is being neglected, abused or in anyway physically or emotionally endangered. It is irrelevant whether the 16-year-old is agreeable to the action or not.
