I don't know where you got the number '12' for a relay, as all the relays in the box of my 2000 Dodge caravan are named for their function. Same in the Dodge workshop manual, all relays are named. In the vehicle the functions for each relay are marked under the box lid, very clearly.

when a blower motor only works on high, it is usually due to faulty resistor board. Should be located in the duct near the blower. the purpose of this is so the air blown by the blower keeps the resistors cool.
