Most of the heroin that comes into the USA comes from either Colombia or Mexico. Dominican street gangs distribute the heroin on the east coast and Mexican gangs control most of the Midwest and west coast. The Mexican heroin is black tar heroin which is a cruder, less potent form than the white powder heroin that comes from colombia

Heroin is not grown. It is created however by processing opium poppies. These are grown in many places but mostly Afghanistan.
