The original location was on the driver side of the engine, just above the starter. Dodge/Cummins changed the parts to the new style goes in the fuel tank.

Usually the injector pump has failed. You will also want to check the lift pump.
My truck is also a 1999 3500 and the press. is about 13 - 14 psi before the filter on a stock lift pump,after the filter you lose about a pound depending on how dirty it is.

The injection pump is in the driver side front of the engine. The lift pump was originally on the driver side of the block near the starter. Chrysler and Cummins released a retrofit kit that moves the lift pump into the fuel tank.
The 2003 has an electronic lift pump. The system is self priming if the lift pump is working.
