I am only speaking about a 1994 Ford XLT but I think the range goes from 1990-1999 on this part. You need not listen to the parts store at the local Napa, etc. The part can be ordered on eBay or your local Ford dealership. The old part number is a Motorcraft F4ZB-13350-BB located right under your steering colum. The part slides off a rail system and its a square blue relay switch with 6 pronges in it. You will need a flathead screwdriver to carefully separate the piece to pull the relay switch out. Once that is removed you will need to order the Motorcraft #SF-631 or 5L5Z-13350-AA it has five pronges due to being an updated part. Place this back into your truck and your turn signals and hazzards work great.
Just a quick note-If your old relay switch smells like your old college days of cooking the relay switch is probally toast.
