Inside the cab on the drivers side next to the emergency brake. You must remove the cover next to your left foot against the cab. The reason i know, I had mine replaced.

yes, but they are slightly different. in 1995 & 96 Ford put a crease at the bottom of the door. this is strictly cosmetic and doesn't affect the functionality of the door.
the model year 1992-1996 the tailgates and body styles for the f150, f250, and f350 were that same and the 1997 f250 and f350 were the still the same design. The f150 however were not. so in short it will fit a 92-96 f150, or a 92- 97 f250 or f350.

As I have a trans in my 92 F150, which has that code on it. I believe my factory service manual identified the trans as a MR2 (or R2) mazada 5 speed that was used by Ford.
Ford F150 can have either one or two converters. Depends on the year and model, I have a 92 F150 Flareside 5.0 and it has 2 catalytic converters. My 03 F150 supercrew has two. One at the back of each exhaust manifold. The one from the drivers side enters a crossover pipe that T's with the passenger side at the back of the transmission on the passenger side.
