The rear reservoir is part of the front reservoir with a separate pump. The pump is located at the bottom of the reservoir closest to the left front fender. The hose then goes through the firewall up over head between the roof and the headliner then comes out on the upper left hand side of the rear hatch. Hope this helps

No, the front and rear washers are fed from the same reservoir. There are two pumps on the reservoir tank. The higher one is for the back window, which works like a warning of low fluid level- if the rear washer doesn't work, it means your fluid level is low and needs to be topped up.
That all depends on where it is leaking. If it is not leaking the hose does not need to be repaired, the problem is elsewhere.
Windshield washer hose is attached on one end to the windshield washer reservoir pump and the other end to the spray nozzles. It is the same as every other car built in the last 50 years. --Ken

There are two pumps underneath the reservoir in the engine compartment, one each for the front and rear washers. The hoses attach to the pumps.
