Islam; in its general sense; means full submission and surrender to God, the Creator and to His will. In this sense Islam started by start of universe creation. All God creatures were created on full submission (Islam) to God. All God prophets called their people to Islam; including Noah, Abraham, Moses, Jesus, and Muhammad (peace be upon them). Islam per Torah revelation to Moses is called Judaism. Islam per Bible revelation to Jesus is called Christianity. Islam per God revelation of Quran to Muhammad is called the same name Islam.

Accordingly, the first Muslim Community; per Quran revelation to prophet Muhammad (PBUH); was in Mecca (Makkah) in nowadays the Kingdom of Saudi Arabia. However, the first Muslim community in a Muslim state was in Medina (also in Kingdom of Saudi Arabia).

Islam is an Arabic word that means surrender to God and worshiping Him as the one and only one God, the Creator. In this sense Islam started by start of universe creation as all God creatures are created on Islam (submission) to God; their creator. However, if you means Islam per revelation of quran by God to prophet Muhammad through the angel Gabriel (Jibril), then it started in Mecca (or Makkah); that is currently in the Kingdom of Saudi arabia; by start of Quran revelation there. Then continued in Medina (also in Saudi Arabia) until the death of prophet Muhammad (PBUH) in year 632 AD. Refer to related question below.
