I believe it was the Presbyterian Church, the Methodist Church, & maybe the Anglican Church that got together to form the United Church.
The Uniting Church in Australia is the third largest denomination in the country. It was formed on 22 June 1977 from the uniting of the Congregational, Methodist and Presbyterian Churches in Australia. Some Presbyterian and Methodist churches still remain independent outside of the union. In uniting, the members of those bodies testified "to that unity which is both Christ's gift and will for the Church" (Basis of Union, para. 1). Ecumenism remains a vital aspect in all of the Church's life and work - in local congregations, national commitments to work together with other churches, and relationships and partnerships with churches of various denominations in Asia and the Pacific.
The church that Jesus Christ left on earth, was the Christian church, His Church, founded by his apostles. But what churches are these exactly. 1. The Church of Rome founded by the Apostle Peter and Paul 2. The Church of Constantinople founded by the Apostle Andrew 3. The Church of Alexandria founded by Mark the Evangelist 4. The Church of Jerusalem which was founded by Jesus himself, the continuation of his work in Jerusalem. 5. The Church of Antioch founded by the Apostle Peter. Each of these five churches before the East-West schism, formed the One true Holy and Catholic Apostolic Church, and it is from these that all other Christian denominations find their routes. Over time these churches witnessed internal schisms such as the protestant reformation which gave birth to the protestant churches. There are a great deal more than seven Christian churches mentioned in the Bible. The term 'church' of course means a group of Christians and not a 'church building', as these had not yet come about, Christians meeting in each others' homes or in secret for fear of persecution. The seven churches that are mentioned in Revelation are: 1. the church at Ephesus 2. The church in Smyra 3. The church in Pergamum (Pergamos) 4. The church in Thyatria 5. The church in Sardis6. The church in Philadelphia 7, the church in Laodicea.However, there were many more churches set up across the Mediterranean area in addition to these. Paul wrote letters to the same church in Ephesus as mentioned in Revelation, but also to churches in Rome, Corinth, Galatia, Philippi, Colossi and Thessalonika. We also know that there were probably several churches set up in Israel, Galilee and Judea, including Jerusalem.

The seventh day adventist church is probably set aside because it is completely different from the other churches. They go by EXACTLY what it says in the Bible. Read: Genesis 1:1 Exodus 2 (just find the sabbath part) Exodus 20:1-4 Revalation (as much as you can)
The many Baptists churches that cater to the blacks. The African Methodist Episcopal Church is the largest of the Methodist churches. There are over 1 million black Catholics as well.

The churches of Christ (Romans 16:16 the churches of Christ salute you) were founded by Jesus Christ ( Matthew 16:18 "upon this rock I will build my church"). The phrase churches of Christ as used in the Bible is a descriptive phrase describing the church as belonging to Christ. The church is his body Ephesians 1:22, 23 and he is the savior of that body Ephesians 5:25. The Churches of Christ are congregational assemblies of New Testament Christians. Their history and background comes out of the 19th century Christian Restoration Movement in the U.S. Alexander Campbell, Barton W. Stone and Thomas Campbell among others were leaders in this movement. Their intent was not to begin a new denomination or church but to restore simple New Testament Christianity. Though Alexander Campbell is often credited by others with founding the Churches of Christ, members of the movement of churches of Christ look to Acts 2 and the day of Pentecost as the beginning and founding of the church.
1 Corinthians 12:12-31 speaks about the body of members that are in Christ. Because the "Church" is considered as the people who encompass the church and not the building itself, it is only logical that this would be the the first mention of the New Testament Church.
What is your definition of "church"? if you are referring to a building made by humans, No! he did not build a church building. (Mt 16:18 "...I will build my church") He is building His church, which is composed of every disciple, forming one body in unity (1 Peter 2:5). The church is the body of believers when they come together to worship and praise God's name in the name of our Lord Jesus. It may be in your own house, park, beach, mountain, etc...
Many Christian churches are doing baby dedications in-lieu of infant baptisms (see 1 Samuel for Hannah's story). Could not find a list of them you will need to ask a particular denomination/church for yourself. I have not heard of Catholic or Episcopal churches doing this service.
