It depends on your age and medical history. A healthy adult eating a good diet including meats does not generally need to take vitamin B-12.

However, the elderly often have trouble with B-12 deficiency and should supplement. Other people are pregnant women, as B-12 is vital for fetal neural development, and vegans/vegetarians as the B-12 found in vegetable products such as soy is not digestible.

People with digestive issues or with pernicious anemia should supplement. So should people who have had stomach surgery or gastric bypasses.

Some medications like antacids and migraine pain medication can inhibit B-12 absorption as well.

People with stomach/digestive issues should not orally supplements, as the digestive tract most likely won't be able to break it down. Instead they should get vitamin B-12 shots or use transdermal vitamin B-12 patches (easier, less painful and cheaper than shots).
