No agreements or contracts or forms of any kind are signed when one becomes a member of the Church of Jesus Christ of Latter-day Saints (the "Mormon" church). To become a Mormon, one is baptized. Baptism is considered a covenant or contract with God, but there is no physical paper form involved. The only official paperwork involved with becoming a member of the Mormon church is a baptism certificate signed by your Bishop. While there aren't any agreements that are physically signed, Mormons believe that by being baptized, you are making a covenant with God.
