Yes, he moved his family into the second floor of the White House following his March 4, 1861 inauguration. He lived there until he was assassinated on April 14 (died on the 15th) of 1865.
He was born in Hardin, Ky. At 9 years old he was moved by his family to Spencer County, Indiana. At the age of 21 he moved to Macon County, Illinois. One year later he move to Coles County, Illinois. At the age of 35 he moved to Springfield, Illinois which was his home base from then on. Of course as President he lived in the White House.

The wife of the 6th president was Louisa Catherine Johnson Adams. I am not sure you would call her the 6th first lady, since the 3rd president did not have a wife when he lived in the White House and the term "first lady" was not commonly used for the president's wife until several years later.
