Well, the compressor might be loose, so when it's engaged, it shakes and thus shakes the entire engine. Or, you may have a loose or broken motor or transmission mount. If this is the case, you would commonly feel some shaking that doesn't feel right and this shaking could be worsened by the compressor being engaged.
Because the motor running the compressor is generating more heat than the compressor is cooling. Once you close the door the space being cooled is smaller and the compressor can keep up and overcome that heat from the motor.

Yes it can. If the fan is not coming on you could have a bad fan motor, a bad capacitor, a loose wire at the contactor, or a loose wire at the capacitor.
