You should have wheel alignment done on your vehicle annually or whenever your wheels feel to be out of alighment while you're driving. If it's something you can feel that your car is out of alignment it is important to have it looked at and corrected as soon as possible.

Alignments the manufacture suggests every 6 months. I work at a place that does alignments and I only do them if I see irregular tire wear (might be too late at this point), or if the truck pulls, but the alignment could be out and the vehicle goes straight down the road. Wheel alignment should be done by a professional shop. Some places (Town Fair) will not align the rear wheels on a Beretta/Corsica due to ridiculous shop policy, but they canbe done. First, ask around to see if a shop near you installs shims for alignment. Shims of proper size/thickness will need to be installed in specific areas behind the wheel bearings in order to properly adjust the rear wheels. If alignment is extremely out-of-spec, you may have a bad wheel hub, or a bent rear beam. The shop will tell you if this is the case, as they should be replaced.

At the bare minimum, you should have it done every time you change tires. But if you see any unusual wear on your tires or any kind of vibrations or pull at the steering wheel, you should have it done right away. Check for wear patterns when you do your regular tire rotation.

depends how far the camber and toe is out on the rear. rear requires shims the front toe and camber can be adjusted in front just have it aligned
When I took my 96 Caravan to be aligned they said there is no rear adjustment that can be done.
It depends on the vehicle and what adjustments need done. On a newer alignment machine the angles can be measured in under ten minutes. Adjustments will vary up to a hour or two.
