In certain conditions, such as cardiovascular disease or following joint replacement surgery, the danger of forming large clots in the blood stream is greater than the danger of bleeding.

Larger clots can block blood flow altogether in major blood vessels-- the larger the vessel blocked, the more tissues supplied by it, and the more damage done. Leading to heart attack, stroke, pulmonary embolism, etc, large clots are more deadly and less treatable after the fact than the side effects of preventive anticoagulants.
