The field hospitals were overcrowded and unsanitary. The concept of sanitation in medical services was fairly new at the time. Medical instruments could not be completely cleaned between uses. Bandages were not germ-free. Medical gloves did not exist. Surgeons and nurses generally believed that it was most important to wash hands after surgery, not before. There was limited clean water in many camps. There were no antibiotics.
