I would say this is because most Americans assume (often incorrectly) that whoever they are speaking to knows the names of all US states. Hence, they do not feel the need to say the name of the country.
Also, Americans tend to identify with individual states, as a way of differentiating themselves from other Americans.
Americans are not collectively as a whole racist towards "countries". While there are many racists in the US and those who hate anyone out of country, there are just as many in other countries who have a disdain for those who are a different skin color to them or not from their home country; and yes, there are people of other countries who are also prejudiced towards Americans.
