Kafir means one who rejects.
'Kafir' is derived from the word 'kufr', which means to conceal or to reject. In Islamic terminology, 'Kafir' means one who conceals or rejects the truth of Islam and a person who rejects Islam is in English called a 'non-Muslim'.
If a 'non-Muslim' considers being called a 'non-Muslim' or 'Kafir', which are one and the same, an abuse, it is due to his misunderstanding about Islam. He or she needs to reach out to proper sources of understanding Islam and Islami terminology, and not only will he not feel abused but appreciate Islam in the proper perspective.

Regardless of whether Islam as a religion condemns this behavior, derisive name-calling by Muslims of non-Muslims certainly happens. This does not mean that all or the majority of Muslims do it, but a minority. The reasons for it are the same as we see in other cases of derisive name-calling: lack of education and/or wishing to strongly advocate a political view. We see the same behavior among a minority of Americans who use six-letter words to describe African-Americans. Similarly, it stems from lack of education and/or wishing to strongly advocate a political view.
However, derisive name-calling does not rise to the level of abuse. At the most, it is purposeful insensitivity and at the least it is simply callousness.
There is a difference between calling someone a Kafir and calling someone a Non-Muslim. Non-Muslim carries no negative word association. It is a neutral word. Kafir is used as a derogatory insult even against Christians and Jews who DO believe in the same God as the Muslims. It is not a neutral or acceptable term in a non-Islamic context. If a person wishes to discuss Atheism or true non-belief in a modern context, the proper word is Mulahed (ملحد). If a person wishes to translated Non-Muslim into Arabic, the word is Ghayr-Muslim (غير مسلم) which most Non-Muslims use. The reason that Kafir is different is because Kafir (كافر) means "denier" or "negator" since they are denying or negating the truth of God. This is an actively negative action. The idea is that Non-Muslims truly know that Islam is the right religion, but conspire to cover this up and present false religions as truth to confuse Muslims. This incenses Non-Muslims, because they actually do believe in the truth of their religions and do not hold any beliefs because of how Muslims may react to them, but because of their honest belief.

Most of us don't but there will always be a few ignorant and or hateful people you can't do anything about. It also depends on what you are defining as "abuse". The American understanding of freedom of speech includes defamatory and hateful forms of protest. Burning of Qur'ans, hateful language, and blasphemy are all permissible under the freedom of speech. However, many Muslims see this as emotional abuse. Freedom of speech does not extend to incitement to violence or to actual physical abuse of people. Americans see no reason to forego the write to criticize and argue against beliefs that they do not agree with. Interestingly enough most Muslims agree with Americans on this point concerning every religion other than Islam.
