Generally speaking most sensors fail because they either burn out from too much voltage or else they simply wear out.

The throttle positioning sensor is located in the throttle body.
DTC P1221 code is: Excessive Difference Between Throttle Position Sensors which means there is something wrong with the electronic throttle positioning sensor system, probably needs replaced
On the throttle body attached to the throttle butterfly shaft.
The only sensors that I am aware of on the throttle body is the throttle position sensor.
