my 1998 Ford Escort has had this problem for a couple days and I went to Shucks and borrowed their fail code sensor for free. when I hooked up the code sensor, it said that our shift solenoid was failing. So, your shift solenoid may have gone out if you have an automatic transmission.
The most common cause for a transmission not to change gears, is low transmission fluid. Check the transmission fluid and fill to the full line.
i have a 95 es300 but it doesnt jerk when i change gears when i ride it jerks but i found out it was a motor mount front motor mount which will cost bout $112 to replace but if when you change gears it could be your transmission slipping that cant be goo but check your motor mounts first....
Did you use the right transmission fluid or did you forget to maintain your transmission otherwise, your gears are broken and needed to be replaced.. chances are you need a new transmission.
