In medieval times, not a lot. Before big-scale industrialization, humanity really didn't have the ability to create much pollution. Cities could be rather unsanitary, and there would be smoke from wood fires, but any other influences would have been very local.
Poeple did not understand the relationship between polutants and health. leading to major epidemics, industrial diseases, and alergys.

Well if you throw rubbish on the ground eg:apple core,orange peel, crisp packet etc.... after a while the litter will decay giving off pollution of some sort. An example would be land-fill_it smells,looks disgusting and can at times give off toxic fumes. -Penguin52738- (Nick Baston)
