One possibility is a heavily sludged or inoperative Idle Air Control(IAC) valve. Get a can of carburetor cleaner and disconnect the air snorkel from in front of the engine. Start the engine, and while using one hand on the throttle lever, keeping the engine running, spray carb cleaner into the open air intake port. Dont spray too much or too little, just try to keep from killing the engine as you do. Spray as much in there as you like, the whole can if you want to. After you are done shul off the engine, and disconnect the battery for about 5 mins, to force the PCM to forget all learned idle stradegy. Reconnect the battery, start the vehicle, and run for a few minutes to allow the PCM to relearn the base idle. Next time you start the engine cold, hopefully your problem will be gone. If not, then it wasn't a sludged IAC.

You get maximum spark when the engine starts to crank.After that the starter motor sucks the energy away from the plugs.So try one second bursts on the starter without touching the gas pedal.If the engine doesnt try to fire,pump the gas pedal once between starting attempts.You may have to repeat this a few times.The trick is to give the engine just enough gas to start.It may not like too much. Once the engine starts from cold it should idle a bit fast until it warms up and then automatically slow down.If it doesnt do this then you have to suspect something wrong with the auto choke.
A bad relay switch could be the culprit if dash lights remain on after engine starts in a Toyota. Cold weather can also cause these lights to stay on after the engine starts.

when the engine starts and is warming up the idle is supposed to be at around 1500 rpm but when the engine warms up the idle should drop to between 700 and 1000 rpm. if it doesnt drop then see a mechanic because it is a very complicated fix
sounds like starter trouble could be the solenoid or the sprag clutch(bendix)
