If you can't do that, then it is only merciful to break off the engagement now. You will have to think about the person you cheated with. Is that person likely to keep silent, or will they blab and cause trouble later on? If they can't be relied on the remain silent forever, you owe your fiance to tell him/her NOW, so that nothing will suddenly come back and bite you both later on.

If you cheated with this ex-coworker and doing this then he is jealous of your relationship and yes it is blackmail/extortion. Unfortunately, if the above mentioned did happen your fiance leaving is not a surprise as you did cheat however, if this was before your fiance and then you must tell him. If he doesn't believe you then there is a trust issue already broken which is hard to repair. You need to deal with this ex-coworker as it seems he has some issues and may not just go away - this is sick behaviour that could get out of hand down the road and for your safety I would suggest going to the police . As for your relationship all you can do is tell your fiance the truth, how you feel and take things from there. Be careful.
