Have you tried replacing the o2 sensor? Perhaps the thermostat as well. My 94 has 94000 miles. My O2 sensor was bad causing the engine to run too lean......sounded like it was choking itself out. Also, my thermostat needed replaced and that too helped it run better.
There is no idle ajustment on a TBI, It is precailbrated and sealed from the factory DO NOT FULL WITH IT AT ALL..... You have another problem like a vaccum leak under the throttle body, are some where.
check for vaccum leaks.
A major vaccum leak.
sounds like the timing is off
On the right side of the throttle pedal
On my Chevy truck it idles high when its low on antifreeze but if its not that more than likely its usually a vacuum line loose or cracked. good luck.

Chevy did not make a pick you p truck in 1941.
If your truck has a "throttle position sensor" then that's where I'd start. The fact that it's an intermittent problem is a good clue that it's a sensor going bad. Also, it won't necessarily trip your "check engine" light. Mine didn't.
