I had the same problem with mines,better yet i still do but mines the door hindge is broken,just the top part of the hindge.so ever time i try to open the door sometimes its okay but most times it comes out ot place so i have to try to put the door back by holding it up and pushing it back in.but as far as for your question,i suppose you have to take the whole door apart.im not really sure,i just have so many things that's wrong with my van its not even funny.I actually have a 96 Chevy Astro van and from the body of the van to the engine it needs some serious maintaince and body work.But it runs good still.

Replace the door latch.
in my 88 astro van I had to completely break out the back handle to first get the door open then I was able to remove interior door cover to do thr replacement. in my 99 asto van I was able to remove the interior cover to open the door then replace the handle.
