Check the blower motor resistor/relay pack usually under the hood connected to the blower motor. You can start by using a multimeter and see if you are getting voltage to the blower motor electrical connector. It should read between 12 and 15v. If not then most likely the resistor/relay need to be replaced.
most common problem is the blower resistor In the firewall on the passenger side of the truck is the blower motor speed resister coils, they burn out alot. I could not find one for my Dakota so I got one from a dodge caravan and I changed the wirring, that is the most likely problem

As for why it blows only on the high speed you most likely will have a bad blower resister and for the noise, it could be a bad blower motor, bad blower wheel or trash that is stuck in the blower wheel or housing.......
Assuming you're referring to the HVAC blower, and not the engine fan, it could be a bad relay, the blower motor could be seized or a solenoid burnt out, it could be bad wiring, it could be a problem with the control switch.

have you checked your fuse? If it won't blow out the air it could be your blower motor.
I had the same problem and ended up replacing the blower resistor and the blower motor. Although it could be one or the other .. mine was both.
