The Vice President is the President of the Senate, although he is allowed to vote only in the case of a tie vote.
If the president is not present, dies, resigns, or is otherwise unable to fulfill his or her duties, the vice president will generally serve as president. And of course, this might depend what country you live in. But for the most part this is correct.
If the President dies (or is permanently incapacitated), the Vice President succeeds to the Presidency for the remainder of the President's term. If there is sufficient time involved, a new Vice President will be nominated to serve the Vice President's term. If the President is alive but temporarily incapacitated, there is a complicated procedure for allowing the Vice President to act as Acting President temporarily, as spelled out in the 25th Amendment to the US Constitution. The amendment considers the cases where the President voluntarily yields his powers or when he does not, as well as the process of returning the powers to the President if he recovers.

The 25th amendment to the Constitution did. It was ratified in 1967. It says that if the president is removed the vice president will become the president.

This is yet to be determined.
The Vice President and the Senate would need to vote to decide if the president is disabled enough that he can not serve anymore. The president can also resign if he feels he can not serve anymore.
