That depends on whether you own the site on which the page is placed, or not.
If you do own the site you have a few options:
Delete the page, then ask for it to be removed via the Google Webmaster interface.
Block indexing of the page with a robots.txt file and then ask for it to be removed via the Google Webmaster interface.
If you don't own the site:
Ask the site owner to remove the page. If they are using your content, name or picture without consent then you could (possibly) seek legal action.
