A useful answer:In most rear wheel drive vehicles 4 th gear is "direct" drive. This means the gears and bearings on the counter shaft are under no load. In all other gears the power runs through the counter shaft.This means the bad gear or bearing does not make noise.
The transmission has a serious internal problem and needs to be repaired.

I assume you are referring to a manual transmission. Grinding while changing gears is an indication of a synchronizer going bad. Until you get it fixed, try double clutching when shifting.

They do if it is an automatic transmission. The Prelude (except Si) has the same manual transmission as well. The Prelude Si has a slightly different manual transmission, but it will still fit into a 1986-1989 Accord.
