Spell Reclamation does not 'attach' to anything. You activate a spell card, and chain Spell Reclamation to it. So;
If Malevolent Catastrophe is chain link 1, the spell is chain link 2, and Spell Reclamation is chain link 3, then Spell Reclamation will resolve first, your spell will resolve second, then Malevolent Catastrophe will resolve last. Instead of going to the graveyard, your spell will return to hand instead.
If your spell is chain link 1, Spell Reclamation is chain link 2, and Malevolent Catastrophe is chain link 3 (it's valid at any link in the chain as long as it's in response to an attack declaration) then Malevolent Catastrophe will resolve and destroy all S/T cards. Spell Reclamation will resolve without effect, as the card it was trying to affect is already in the graveyard. Lastly chain link 1 will resolve correctly as normal.
They do. They go to the graveyard first, and then 'disappear', meaning anything that triggers when a creature is destroyed, will trigger from seeing a token be destroyed.
A card that is destroyed is 'sent to the graveyard', but sometimes a card instructs you to 'just' send a card to the graveyard, and this is not destruction.
Obviously into the graveyard.
Flint is sent to the graveyard
This would depend on the wording of the card's lore. If the card states that "...when this card is sent to the Graveyard..." or "...if this card is destroyed by a card effect...", then it would imply that the effects are activated when it hits the Graveyard. However, if the card states "...when this card is destroyed by battle...", then this would imply that the effect would only activate if it attacked or was attacked and destroyed during the Damage Step. As for Flip Effect Monsters, their effects would only apply if they were flipped face-up before they were destroyed. If not, then their effects are not activated.

The monster will always return to its owner's Graveyard. Incidentally, if the effect of the destroyed monster activates while it is still on the field, you will be able to use it; however, if the effect activates in the Graveyard, the card's owner will control the effect.
Destroyed monsters go to the graveyard, including destroyed Fusion or Synchro monsters.

the reclaiming of land that was at one point forest but gave way so something could be put in its place. the act of forest reclamation refers to restoring a forest that was essentially destroyed.
Destroyed or tributed Fusion Monsters go to the graveyard. They will return to the Extra Deck if something tries to return them to hand or deck.
Flint is sent to the graveyard
Yes they do, as they activate their effect when they are destroyed and sent to the graveyard.
No, Equip Cards simply 'fall off' when the monster they are equipped to is destroyed. They do not go to the graveyard, they are now just unattached.
