For Spain it had very few ramifications other than confirming that its leadership was ineffective and problematic. This would lead to the eventual abdication of King Alfonso XIII in 1913 (only 15 years later).

As concerns the USA, Cuba, Puerto Rico, and the Philippines transferred over to US Authority. This made the United States an Imperial Power and (in many American's minds) on par with the European Powers for the first time. This also expanded America's Spheres of Influence and access to the Far East. The heroes of the Spanish-American War became influential political figures (Teddy Roosevelt even becoming President of the United States).

As concerns Cuba and the Philippines, the United States made very strong requirements of these two nations and governed them in a style very similar to the colony-system that Europe had previously pioneered. The assumption always was that Cuba and the Philippines would be eventually let go, but no timeline was ever set. Filipino Revolutionaries like Aguinaldo who had helped the USA against the Spanish, began to attack American interests after the War.

Puerto Rico was more fully integrated into the USA because there was no intention of every releasing it from US Authority. Life improved somewhat during the century of American control.
