The Roman Republic began to fall apart years before it finally fell due to the cronyism and unyielding attitudes of certain members of the senatorial class. Powerful families made alliances more for their own advancement than for the good of the state. Elections were often rigged. Tribunes bought. By the time Julius Caesar came to power, the senate was split into two main groups, the Optimates, who wanted things the way they had always been, and the Populists who were for change. By taking advantage of this split, Caesar was able to rise to the position of Dictator and then Dictator for Life, brushing aside the authority of the senate and the Roman people. After Caesar's death the government retained the appearance of a republic, but in fact, was controlled by the triumvirates and their factions. After Actium, Octavian/Augustus transformed the republic into the principate.
The Roman Republic fell because it was a Republic unpopular with its citizens, which meant its army didn't support it, and was just waiting for a chance to rebel. That chance came with Julius Caesar.
There were several reasons for the fall of the republic, but the main one was corruption. The senate was divided into two groups, the optimates, who were stubbornly entrenched in the old ways and adverse to change, and the populists, who advocated change in order to keep up with the times. Each group bribed their way into authority so they could put forth their own agendas and further their own concerns. The good of the state took second place to personal advancement.

The most commonly used date is 476, but other dates are also used. Some historians just say 5th century, or the second half of the 5th century. Clearly it was a long, drawn out affair. The East Roman Empire survived until 1453. The name we use for it is the Byzantine Empire, but that name was never used at the time, and they were nearly always called the Roman Empire during the Middle Ages.
