Believe it or not, not very many. There was this train wreck on Big Thunder Mountain Railroad in Frontierland, but that was about 30 years ago. The ride has been revamped since then. Also, there used to be these air rides above Fantasyland, before the mono rail station was inveted. But then some stupid people jumped out of them to commit suicide. So they had to be removed. So all together, roughly under ten people. I could be wrong about the numbers though.
Yes, a few. Most of them were doing something stupid, like trying to transfer between moving ride vehicles or entering a dangerous area despite Fencing designed to stop that sort of behavior AND posted warning signs, but a few people have died due to negligence or error on Disneyland's part.
