Being trapped in the car while it was filling up with Ice cold water Elena couldn't breathe or stand the cold. Water went into both her mouth and lungs which caused her to die. In time that she died Stefan pulled her out of the water and left her under a willow tree. That is where she later on wakes up to find out she's a vampire.
