An uncontrolled release of hydrocarbons ignited on the drill floor. Primary well control was provided by the completion fluid, but this was being changed out during normal testing operations. Primary well control was then the cement job on the liner which was woefully inadequate in design (BP US) and execution (Halliburton). With no primary well control, then the rig relied on secondary well control, the blowout preventer (Transocean). This also failed. Due to the explosion, damage and loss of the Deepwater Horizon there was no way to recover control. Therefore, tertiary well control was implemented; drilling a relief well into the toe of the uncontrolled well and killing it from the bottom.

The accident investigations are still ongoing. However, BP was ultimately in charge of the drilling of the well, so it bears primary responsibility. BP has never denied that they were at fault, but has also said that others, primarily the owner of the drilling rig, Transocean, also made judgment errors which contributed to the accident.

BP stands for British petroleum the invigorate means its a nitrogen enriched gasoline. They do this because they say it clean your engine thus your car will be more efficient and get closer to it best possible gas mileage. However if u plan on red lining your car or just like driving your car with high rpms this kind of fuel will probably blow your engine faster than normal gas

During the clean up operations, there was many suggestions of mopping up oil using sponges, mats made of hair, and maybe sawdust. Any absorbing material should be able to remove some oil from water. Small spills of oil or other pollutants in calm water, may be more effectively cleaned up using these mats. Material or mats that absorbs oil must then be picked up and creates a very large disposal problem. Skimmers seemed to work best.

There was an explosion, the pipeline split and oil gushed out into the sea, so the effect of that was the oil rose to the surface.

The company man on an offshore drilling rig has the responsibility to supervise the execution of the approved drilling program, including all revisions made by drilling engineers. He is the top representative of the oil company which has hired or contracted other companies to drill the well.

In the case of the BP oil spill, BP contracted Transocean for the rig and drilling of the well. BP contracted Halliburton to cement in the well. Each of these contractor companies keep the company man informed of what they are doing, and if they have problems, the company man must be informed and has final approval authority.

In BP's case, and generally in most drilling operations, there is a morning meeting between the rig and the land based office, where the days activities are discussed.

This is a simple question. The answer is not simple. There are a number of investigations that have not been completed, including a National Commission that includes many of the countries top scientists and engineers.

As we learn more about potential contributing factors leading to the BP oil spill, there will be efforts to be sure these errors are not repeated. The contributing factors include (1) the failure of the cement job to seal the reservoir from the production casing, (2) the failure of the rig crew and well leaders to identify that there was a problem with well integrity during the negative test, and (3) the failure of the blow out preventers from sealing in the well.

So, while the investigations are still incomplete, it does seem to indicate that better design and human judgments could have lowered the chance of an accident.
