All major airlines have frequent flyer programs. They track the flights that program members take. After a certain number of flights, members are eligible for awards such as free flights or upgrades.

Because many people have the same name, and addresses change, airlines assign each member of their program a number. (It may contain letters, but it's commonly called a "number" anyhow.) This is the person's frequent flyer number or flyer number. If a person is a member of more than one program (you might as well be if you travel a lot, it costs nothing to join them) the numbers will be different for each airline.

No unless you have high elite/medallion status with an airline. You could then cancel your paid flight and get a full refund and then rebook with miles. You can do this as a passenger without Frequent Fier status too, but it would cost quite a bit of extra money.

The upgrades go in segments, where each segment is determined by the number of miles that the flight is. Thus, you may be able to upgrade for a few thousand filght miles if flying from New York City to Boston, but it will cost you a lot of miles to upgrade if flying from New York City to Tokyo. Depends on the type of ticket. For 25,000 miles you can get a round-trip economy class ticket within the U.S., assuming the flight isn't very popular with award travelers. If the initial allocation of award seats is gone, if you want to fly in a first or business class, or if you want to fly to another part of the world, you will need more miles. Delta finished the merger of the Northwest Worldperks FFP into its existing Skymiles FFP on October 1, 2009. At that time, one of three things happened. Users with a Worldperks account but no Skymiles account had only a Skymiles account going forward. Users having both who had previously responed to the request to link them had their Worldperks account merged into their Skymiles account. Users with both accounts that didn't link them ended up with two Skymiles accounts and had to merge them manually.
