Forbidden Forest: when you restart the level in the circle go straight up (top of the screen), avoid the dugbogs, past the mushrooms, and there it is.
Prefect's Bathroom: restart level in circle. go down to the left, past the salamanders and it should be on the other side of the bridge.
Herbology: When you restart the level in the circle, go back up the steps into the greenhouse, through the lily pads again heading to the right of the screen, back to the small greenhouse. Make sure you use a character with the Herbology card to grow the lilies faster!
PS. All the vanishing cards disappear a little while after you've entered the level (maybe 1 minute or so), so you have to be fast
