It usually means the internet connection somewhere between you and the server has a slow or bad connection or the computer isn't powerful enough to keep up with all the connections.

If your playing an the same computer that's hosting a server with 3 or 4 other players it can overwhelm it. You either need a faster internet or you need a dedicated computer for your server.

A Lan party with many connections are no problem, it's just the internet connections that can hang things up.
