That would primarily depend on whether they retired as an enlisted man, or as an officer. It would also depend on your familiarity with the individual.
In general, you would just refer to them as Mr. Whatshisname, or Mrs. Whatshername.
Exceptions are usually for Field Grade Officers (Major, Lieutenant Colonel, Colonel) and General Officers (Brigadier General, Major General, Lieutenant General, General). In these cases, you would use their working title:
Major : Major Whatshisname (Abbreviated MAJ)
Lieutenant Colonel : Colonel Whatshisname (Abbreviated LTCOL)
Colonel : Colonel Whatshisname (Abbreviated COL)
All general titles : General Whatshisname (Abbreviated BGEN, MGEN, LGEN, GEN)

Depends on what you are writing her name on. If you are writing a letter to her and she is a personal friend than you address her as you do in speech. This would probably be just "Mary" or a nick name that you normally address her by. If you are referring to her in a written correspondence and the party that you are corresponding with is not a personal friend of her's or yours than you would refer to her as Mrs. Smith or Mrs. Mary Smith. Generally the only time you use her husbands name is if you are introducing them together as in "Mr. & Mrs. Bill Smith" but that has become a bit archaic and I often here people introduce couples as "Mr. Bill and Mrs. Mary Smith" or "Mr. and Mrs. Smith"

Traditionally a general never really retires. They may retire from active duty, but they can be called back to service anytime by the President. Historically, military officers always kept their rank as a title but the idea fell out of favor after WWII, but it is still common and proper practice to address flag officers as general or admiral depending on their service branch.
A Ph.D., or an equivalent, is considered a professional designation and generally the people that hold them only use the letters or are styled as doctor in their professional lives. Also a person who holds a doctorate is never styled as doctor in written form, only verbally. When writing their name the proper way would be to use the initials after their name. Though it is also incorrect, to write Dr. Smith implies they are a medical doctor.
If writing specifically to the wife you would write:
