Of Hawking's many contributions to theoretical physics, perhaps two stand out. His work on discovering and defining black holes has allowed us to explain much of the missing information about how the universe started and about it's ultimate fate.

Secondly, his proposal of matter-antimatter pair anhillation near a black hole event horizon and his conclusions that black holes must actually glow slightly (more glow for smaller black holes) is one of the best scientific theories that tie together Einstein's theory of Relativity and Quantum theory. These two great breakthroughs of the 20th century are often in conflict and this is a cause of great irritation and embarrassment to physicists (who hate to be wrong or even partly right). So, Hawking's work is seen as a paragon because it is consistant with both theories.

The voice synthesis machine Hawking used through his life (althought not in the last year) is a DECTalk DTC-01. The emulator for it is in the MESS project (http://www.mess.org). You must also obtain a ROM file separately, I can't link it here.

Anyway, I tested it and it sounds just like Hawking, albeit with a few minor pronunciation differences (prolly because he uses a different revision of the machine). The basic idea is that back in the 80s he had just enough movement ability in his hands due to his disorder called amyotrophic lateral sclerosis. He would use slight movements of his hands to move cursor on a screen which helps him build sentences. And you know how when you are typing a search into google and it often assumes what you are going to type? Well, his computer does something similar but more advanced in forming sentences so that he did not have to make more slight movements with his hand than he had to. He used to use a machine called the DecTalk DTC01 voice synthesizer.

He has since upgraded to the more advanced NeoSpeech VoiceText speech synthesizer, and also unfortunately has since lost any ability to make slight movements with his hand. So the previous answer is correct in that he is only now able to move his cheek on one side of his face. His new speech synthesizer allows him to select words and form sentences on his computer screen, but he controls this one by moving his cheek and a sensored attached to his glasses detects these movements. It often takes him several minutes to form a short response when speaking to someone.

The man himself can tell you. Here is a direct quote from Stephen Hawking's website:

"In 1980, we changed to a system of community and private nurses, who came in for an hour or two in the morning and evening. This lasted until I caught pneumonia in 1985. I had to have a tracheotomy operation. After this, I had to have 24 hour nursing care. This was made possible by grants from several foundations.

"Before the operation, my speech had been getting more slurred, so that only a few people who knew me well, could understand me. But at least I could communicate. I wrote scientific papers by dictating to a secretary, and I gave seminars through an interpreter, who repeated my words more clearly. However, the tracheotomy operation removed my ability to speak altogether. For a time, the only way I could communicate was to spell out words letter by letter, by raising my eyebrows when someone pointed to the right letter on a spelling card. It is pretty difficult to carry on a conversation like that, let alone write a scientific paper. However, a computer expert in California, called Walt Woltosz, heard of my plight. He sent me a computer program he had written, called Equalizer. This allowed me to select words from a series of menus on the screen, by pressing a switch in my hand. The program could also be controlled by a switch, operated by head or eye movement. When I have built up what I want to say, I can send it to a speech synthesizer. At first, I just ran the Equalizer program on a desk top computer.
